COVID Testing

CPS has announced that the COVID-19 testing or proof of vaccination requirements for student-athletes will take effect on Monday, September 20 at all high schools. Please read the following instructions:
  1. For students who are fully vaccinated:

    1. Students/families must provide proof of COVID-19 vaccination. We are asking that parents or guardians bring a copy of their child’s vaccination card to the school nurse or school clerk. The school nurse will enter the vaccination dates into the district’s HIPAA-compliant student information system, and the child will be exempted from weekly COVID-19 testing. 

    2. Individuals are considered “fully vaccinated” two weeks after their second dose of a two-dose vaccine series. Students who are in the vaccination process will need to submit to weekly testing until they are fully vaccinated

  • For students who are not fully vaccinated:

      1. COVID-19 testing consent will only need to be completed once per year. Unvaccinated students cannot participate in sports without testing consent. Please find Consent Form instructions for your school below:

  1. Open the COVID-19 Testing Parent Consent Form Link.

  2. Scroll to find your specific school name in the left-hand column.

  3. On the far right side of the document, under the URL STUDENTS tab, please select and share ONLY your respective school’s student testing consent form link. This link can be shared with parents/guardians.

  4. Once the link is open, parents should answer the screener questions as asymptomatic in order to proceed to the consent sign-off page.

  5. Once parents/guardians complete the consent form, the student will be eligible to test in order to play sports.
